    Accident: 100101450 - Dies Of Apparent Heart Attack

    At approximately 2:00 p.m. on September 13, 2000, Employee #1 was replacing a bearing in an exhaust system when he suffered an apparent heart attack and fell from the ladder on which he was working. He was killed.

    Accident: 14217590 - Fire Fighter Dies Of Heart Attack In Sleep

    On June 25, 2000, Employee #1, an accountability officer for a volunteer fire company, and coworkers responded to a structural fire as mutual aid to an electric boat fire department. One engine from the volunteer fire company arrived on the scene and was in stand-by mode. This engine had two volunteer fire fighters and one carrier fire fighter on board. After they had been in stand-by mode for approximately 10 minutes, the engine was released to return back to the station because it was a false alarm. The driver noticed when they arrived on the scene, that Employee #1 was asleep or resting. After they were released, the driver noticed that Employee #1 had not moved from his original position. He tried to wake Employee #1, but he was nonresponsive. The driver then called for assistance from emergency medical services, who were already there. They tried resuscitation efforts, but he was still nonresponsive. Employee #1 had died from a heart attack while in his sleep. He had not left the vehicle or engaged in any fire fighting activities.

    Accident: 170007181 - Dies Of Apparent Heart Attack

    At approximately 7:15 a.m. on June 13th, 2000, Employee #1 was observing operations at a construction site in Spokane, WA. He was walking alongside some equipment when he collapsed. The equipment operator went to help him and Employee #1 said he was having a heart attack. An ambulance was called and transported Employee #1 to Valley General Hospital, where he died. The Medical Examiner's report listed the cause of death as arteriosclerotic cardiovascular disease.

    Accident: 170041206 - Dies After Suffering A Heart Attack

    At approximately 1:40 a.m. on June 9, 2000, Employee #1 was working on the weigh-and-price line at the poultry processing plant in Kelso, WA. She was responsible for transferring packages of wrapped poultry products from storage trays to a conveyor line for final packaging. Employee #1 began suffering severe chest pains and slumped to the floor. Nearby coworkers immediately called for medical attention. An onsite EMT responded and began first-aid and CPR until Emergency Services arrived and took over. Employee #1 was transported by ambulance to St. John's Hospital in Longview, WA, where she was pronounced dead on arrival due to an apparent heart attack. Employee #1 had a prior history of heart disease and high blood pressure. The autopsy report indicated that Employee #1 died of a dissecting aneurysm of the ascending aorta. There were no work-related factors contributing to her death.

    Accident: 170040687 - Dies After Suffering A Heart

    Employee #1, a yarder operator at a logging site, went to the yarder and warmed it up while the crew went to the landing to move some lines. When they signaled for a line movement, nothing happened. The hooker yelled to the chaser to see what Employee #1 was doing. The chaser got to Employee #1's station and found him slumped over in the operator's seat. The crew implemented emergency procedures, attending to Employee #1 and calling for the Emergency Response Group. Employee #1 died after suffering a heart attack.

    Accident: 170206007 - Dies After Suffering Heart Attack

    Employee #1 was using a company vehicle to complete a field assignment. He took a morning break, made a private purchase, and then asked to use the restroom. Employee #1 suffered a heart attack and died in the arms of the restroom attendant.

    Accident: 170205991 - Dies After Suffering Heart Attack

    At approximately 7:06 a.m. on May 4, 2000, Employee #1, a 73-year-old bartender, suffered a heart attack and died shortly afterward.
