    Accident: 14258842 - Injured When Struck By Flying Door

    Employee #1 was cleaning up in front of a mercury retort unit. A fire inside the unit increased the interior pressure and the door blew off its hinges, striking Employee #1. He sustained injuries that required hospitalization. Heavy aliphatic hydrocarbons were found in two metal vessels within the retort unit.

    Employee #1 was inflating a tire when it exploded. He suffered multiple fractures and was killed.

    Accident: 126008903 - Arm And Ribs Broken By High Pressure Water Blast

    At 7:00 a.m. on June 3, 1999, Employee #1 was in an approximately 14 ft deep excavation, replacing a coupling on an 8 in. water line. As he was working, an adjacent 16 in. water line failed and a blast of water struck Employee #1 in the chest and on the left side of his body. He suffered a fractured left arm, four fractured ribs, and a lacerated spleen and kidney. Employee #1 was taken to Kaiser Hospital in San Diego for treatment. According to the foreman, the cause of the accident was the apparent failure of the 16 in. water line at a point where it had previously been repaired.

    Accident: 125626804 - Injured When Struck By Force Of Exploding Tire

    Employee #1 worked for a store that sold and serviced tires, wheels, brakes, and accessories. He was inflating a passenger car tire on a rim-clamp tire changer when the tire burst and he was hurled backward and to the floor. Employee #1 suffered severe bruising to his lower back and buttocks. He was treated at the emergency room at San Leandro Hospital, and released to the care of the company doctor. The employer was cited for an accident-related violation of T8CCR 3326(i)(1), for incompatibility between the tire and rim.

    Accident: 126065168 - Burned By Hot Water Release

    At approximately 8:30 p.m. on November 19, 1998, Employee #1, a maintenance supervisor, was dismantling a high-temperature fault on the CIP system in pasteurizing line #1. He removed the cover without using an air blow valve to check the system. The temperature and pressure of the water increased and the hot water overflowed, striking Employee #1. He suffered second-degree burns on his arms, chest, and neck, and was taken to the USC burn unit. He was subsequently transferred to the Irvine Medical Center , where he was hospitalized for six days and then treated as an outpatient.
