Accident: 170209860 - Slightly Injured In Boiler Explosion At approximately 11:00 a.m. on February 3, 1994, Employee #1 was working in a room in which the motel's main water heater boiler was located when, without warning, the heater exploded. The explosion blew a hole in the adjacent wall, and fragments of the boiler injured Employee #1, but not seriously and not requiring hospitalization. When the water heater's pressure relief valve was removed and inspected, it was found to be clogged by what appeared to be heavy rust deposits, and therefore not in operating condition. He had been drinking prior to the start of the fire. Employee #1 was a careless smoker and had been talked to about leaving burning cigarettes lying around his work area causing many burn marks. Coworkers who had been in his room prior to the fire said that Employee #1 had been lying on a davenport smoking and drinking until the early hours in the morning on the day of the fire. fire, hotel, work rules Accident: 14300560 - Dies Of Carbon Monoxide Poisoning Employee #1 entered a trash compactor room after the hotel's emergency generator in the adjacent room was tested. It appeared that high levels of carbon monoxide had entered the compactor room from an 8 ft by 12 ft space above the roll up door. Employee #1 was found unconscious on the ramp to the compactor approximately 20 minutes after the emergency generator was shut down. He died. Six employees who administered CPR were treated and released. In addition, one fire fighter and two paramedics were treated. Initially, it was thought that Employee #1 suffered a heart attack; however, the autopsy showed high concentrations of carboxyhemoglobin in the employee's body.
