    Accident: 120277280 - Killed By Intoxicated Driver

    Employee #1, a regular employee of a general highway construction contractor, was working within a closed lane on the freeway. He was picking up red traffic cones while standing on a rack that was mounted on the rear of a 1995 Ford F350XL construction truck. The truck was backing down the closed lane when a 1999 Ford F250 automobile driven by an intoxicated driver swerved into the closed lane and crashed into the rear of the construction truck. Employee #1 was pinned between the two vehicles and was killed. The accident was caused by the alcohol-impaired driver. No violation of OSHA standards was found.

    Accident: 14228241 - Cut By Saw Bleeds To Death

    At approximately 1:13 p.m. on June 22, 1998, Employee #1 was trimming trees in the rear alleyway of a row house in Wilmington, DE. The saw apparently kicked back, lacerating his arm and leg. Employee #1 was found positioned on a tree branch, having apparently bled to death. The local police report indicated that he had been drinking alcohol prior to the accident and that a crack pipe was recovered from his pocket.

    Accident: 170097281 - Killed In Motor Vehicle Accident

    At approximately 10:00 p.m. on April 14, 1998, Employee #1 lost control of the vehicle she was operating and was killed. She had been drinking and had a blood alcohol level to 0.26. Employee #1 was exceeding the speed limit and was not wearing a seat belt at the time of the accident.

    Accident: 170243653 - Two Killed In Drunk Driving Accident

    At 5:00 p.m., Employees #1 and #2 were observed at the Starhouse Lounge. At 7:00 p.m., they were riding in a pickup truck driven by Employee #2 when they collided at high speed with the back of a water truck. Both men were killed. Subsequent toxicology reports showed both men's blood alcohol level to be 0.17. It was concluded that the accident was the result of Employee #2 driving while intoxicated. Neither man was on duty at the time of the accident; Employee #2 was the boss of Employee #1 and the owner of the company.
