    Accident: 201270410 - Killed In Fall From Joist When Scaffold Overturns

    Employee #1 and a coworker were using the overhead bar joists to pull themselves along on a 21 ft high mobile scaffold. The scaffold overturned, and they were left hanging from a joist. The coworker managed to climb down, but Employee #1 fell, striking his head on the floor. He was killed.

    Accident: 200550044 - Killed In Fall From Steel Frame Building

    Employee #1 was on the steel frame of a building, repositioning some steel bar joists, when three of the joists twisted. Both he and the joints fell approximately 20 ft, landing on a concrete slab. Employee #1 was pronounced dead at the scene.

    Accident: 200210235 - Killed In Fall From Rooftop

    Employee #1 was welding bar joists onto the steel frame of a classroom under construction. He had climbed onto an adjacent rooftop to measure and space the bar joists for welding. He fell 16 ft 2 in. and was killed.

    Accident: 201090719 - Injured In Fall Through Roof

    At approximately 9:18 a.m. on April 15, 1997, Employee #1 was on the roof of a building. He stepped onto a 2 by 6 joist that was also holding between 10 and 15 pieces of plywood, each of which weighed 20 lb. The combined weight of Employee #1 and the plywood was too much for the joist to support. The joist failed and Employee #1 and the plywood fell 20 ft through the roof. He landed on cement, sustaining rib and shoulder injuries. The employer was cited for failure to notify the OSHA division of the accident, and for a live load hazard.

    Accident: 200210227 - Killed In Fall From Roof Deck

    Employee #1, a steel erector, was working on the third phase of construction, which consisted of a large concrete foundation with upright concrete columns to support the steel I-beams along the east edge, the center, and the west side of the existing building. The steel I-beams that had been installed created eight bays. Bar joists of approximately 40 ft spans had been spread out between the beams. Each end of the bar joists had been welded, with the exception of those along the steel beam on the west side. A coworker began placing bundles of steel roof decking on top of the bar joists along the west I-beam, staggering them to prevent overlapping. A fourth bundle of roof decking had been placed farther out on the bar joists. Employee #1 and the coworker were attempting to receive another bundle of steel roof decking across the bar joists over the fourth bay on the west side. There had been no cross-bracing installed between the bar joists to prevent side movement, and the ends of the bar joists had not been welded on the west side. As a result, the bar joists moved and fell onto the concrete slab foundation. This caused Employee #1 to fall from the roof deck. He was killed.
