Accident: 170008510 - Arm Fractured When Caught In Engine Lathe Employee #1, a lead machinist, was showing an apprentice how to use an engine lathe. Employee #1 was moving the coolant line when his arm became caught in the machine and was fractured. lathe, arm, caught by, machinist, fracture Accident: 121905368 - Killed When Struck By Flying Machine Parts At approximately 5:15 p.m. on May 29, 1998, Employee #1 was operating a Sidney lathe when the safety block and driver system flew off the face plate. The driver struck him in the chest and abdomen, while the safety block was thrown approximately 141 ft and hit a concrete block wall. Employee #1 was killed. The employer designed, manufactured, and used a non-industry standard lathe dog drive and safety block system, which allowed the device to fly apart under either normal or emergency conditions. lathe, struck by, flying object, chest, abdomen, work rules, safety block, equipment failure Accident: 125350710 - Injured When Head Strikes Lathe Part Employee #1 was running an automatic Crown metal lathe with a rotating tool turret when the parts catcher failed to properly catch a part. He reached into the machine to pick up the part up and was struck on the head when the turret rotated. Employee #1 sustained injuries that required hospitalization. He had been running the lathe with the enclosed door open and the interlock off. lathe, interlock, rotating parts, head, struck by, work rules Accident: 171055445 - Injured When Arm Caught In Hose Lathe Employee #1, an apprentice hose builder, was working on his first hose without supervision. The hose was 3 in. in diameter by 12 ft long. He started to apply a rubberized fabric at its head, and then slowly moved the fabric on a bias to a point about 2 ft from the head. A control box was clipped to his pants pocket to control the rotation of the lathe. The box had STOP and START buttons near the top, and a switch below this that turned off the lathe or changed its rotation. Employee #1 was toggling between the STOP and START buttons to slowly inch the fabric onto the hose. He had removed about 6 ft of a protective plastic wrapper from the fabric when he looked down to see if his finger was on the STOP button. His left hand became caught between the fabric and the hose, and he was flipped on the table. According to his supervisor, the control box was underneath him and the STOP button was depressed when he fell, halting the lathe immediately. Employee #1 tore several tendons in his left arm and severely fractured both bones in his left forearm. hose, lathe, inexperience, rotating shaft, caught between, fracture, arm, stop switch, tendon, work rules Accident: 170572887 - Injured In Chain Conveyor, Later Dies Of Blood Clot Employee #1, a veneer lathe operator, was unjamming a log that had become lodged in the core chain conveyor. The lathe had been shut down, along with the core chain. The core chain, which ran the length of the lathe at floor level and operated at 105 ft per minute, takes the leftover core of the stripped log out of the process. Employee #1 climbed onto the chain and was straddling the chain/chute at the end, near the sprocket drum drive. He was using a pipe to loosen the jammed log and then signaled a coworker. His intentions were unclear to the coworker, who activated the core chain. The heel of Employee #1's left boot became caught between a crossover bar and the activated core chain, crushing his heel and part of his foot. He was admitted to the hospital; five months later, Employee #1 died of a blood clot secondary to his injuries.
