LIMB LOSS PREVENTION

Effect of Custom Orthotics on Hindfoot Kinematics of the Foot and Forefoot - Pressure distribution data from flatfoot specimens will be used to determine if the constraint provided by orthotics alters the kinematics of the hindfoot to understand a potential cause of foot ulcers.

Equinus Contracture Measurement in Diabetic Foot Ulcer Patients - Diabetic patients sampled to determine relationship between equinus contracture and plantar foot ulcers. Subject data collection wa completed in January 2000.

A Computational Model of the Human Foot - Development of a sophisticated computer graphic foot model to further understand the function of the foot and its relationship to pathologic disorders. The three-dimensional Euler angle rotations of the calcaneus, tibia, navicular and cuboid were determined. The data was compared to the three-dimensional rotatins of 11 cadaveric foot specimens exercised in identical fashion. The computational model came within one standard deviation of the cadaveric mean for 10 out of 12 rotations and within two standard deviations of the mean of 12 of 12 rotations.

PROSTHETICS DESIGN

Development of Lower Limb Powered Prostheses - The development of an artificial musculo-tendon actuator that mimics the function of the triceps surae and Achilles tendon has been accomplished.

Silicone Suspension Liner/Socket vs. Conventional Socket with Pelite Liner - A comparison of suspension systems using a step activity monitors and a questionnaire to quantify prosthetic treatment.

Characterization of Shock Absorbing Pylons Used in Transtibial Prostheses - Use of mechanical and human subject testing to determine the performance of commercially available shock absorbing pylons to ultimately provide clinical pylon prescription guidelines.

OUTCOMES

Outcomes Assessment of Amputation vs. Vascular Bypass Surgery in Diabetes - A retrospective analysis of VA hospital data to assess the short- and long-term outcomes of interventions for vascular disease of the lower extremity was completed in May, 2000, publications pending.

Quality of Life: Issues of Interest to Lower Limb Amputees - Manuscript published in the July 1999 Journal of RR&D. A secondary data analysis to report recreational and leisure activities of lower limb amputees is planned for 2001.

Survey of Outcome Measures to Evaluate Care of Lower Limb Impairments in Frail, Elderly, or Low Functioning patients - Measures are presented on a grid: World Health Organization functioning and disability categories (impairments to satisfaction with life) by source of information (patient report, provider protocols, technology).

Biomechanical Risk Factors for Low-Back Pain After Above Knee Amputations - Pilot study to explore low back pain in persons with above knee amputations. Proposal submitted in the October 2000 Merit Review cycle.

Vascular Intervention Outcomes of the Lower Extremity - Retrospective review of VA Patient Treatment File 1989-1999 on amputations has been completed. A prospective, functional outcomes study of diabetic and non-diabetic veterans undergoing vascular bypass, amputation, or angioplasty was submitted to VA Interdisciplinary Studies as a QUERI proposal. The QUERI was funded with the title: Impact of a Quality Management Intervention Upon Foot Care Outcomes. It is a 16-month project to be completed in September of 2001.
